·       Experience Modification less than 1.00 – refer higher modification factor’s to company Underwriter

·       Up to 50 employees at one location

·       Minimum Payrolls:

o      $15,000 per Full Time Employee

o      $7,500 per Part Time Employee

·       Loss Experience – no more than 2 medical only claims with paid + reserve of less than $2,500

·       Delivery is limited to no more than 20% of total sales

·       Business should close by midnight

·       Kitchen must be located on same floor as the serving area

·       No Blood Banks, Blood Donor Centers, Traumatic Injury or First Aid Centers

·       Salespersons limited to 25% of total number of employees and 40% of total payroll. 

 

 

  • Submit to company Completed and signed ACORD applications 125 and 130 and rating worksheet
  • ACORD form 125 – Referral to underwriter required for “Yes” answers to questions 1, 2, 4, 5, 8, 9, 10, 11, 12
  • ACORD form 130 – Referral to underwriter required for “Yes” answers to questions 1, 2, 3, 4, 5, 6, 7, 8, 10, 11, 13, 14, 18, 10, 20, 21, 22, 23
